Southwest Virginia Community Health Systems which includes Saltville Medical Center, Twin City Medical Center, Tazewell Community Health, and Meadowview Health Clinic, will destroy inactive health records beginning June 15, 2017 according to the Code of Virginia Section 42.1-79-1. The individual receiving services or an authorized representative will have an opportunity to request possession of their records by calling our office. Proof of identity will be required. Included in the purging will be any adult record inactive prior to July 1, 2007, as well as inactive records of juveniles who are at least 18 years of age, or have been inactive 6 years, whichever is longest. This purging will inactive records (as described above) from all programs administered by Southwest Virginia Community Health Systems, Inc. The deadline to contact Southwest Virginia Community Health Systems is June 14, 2017. Any record not requested by that date will be destroyed.
