Substance Abuse Disorder Healthcare Navigator (Tazewell and Saltville)

Position Overview:
As a Healthcare Navigator, you will play a key role in supporting patients through their recovery journey. You will guide individuals with Substance Use Disorders (SUD) through SVCHS services and connect them to community resources, helping them access the care they need. This role involves outreach to community partners, providing transportation, and contributing to an interdisciplinary team that works together to ensure comprehensive care and support for patients.

Key Responsibilities:

  • Guide and support patients with Substance Use Disorder (SUD) in accessing SVCHS services and community resources.
  • Build and maintain referral pathways by engaging with community partners to enhance patient care.
  • Supervise and provide education to patients engaged in peer recovery services.
  • Provide transportation to patients to help them access healthcare and community services.
  • Collaborate with the New Day Recovery interdisciplinary team to develop and implement care plans for patients in recovery.
  • Help identify and develop resources to support patient recovery, including mental health, housing, and employment services.
  • Assist in care coordination and follow-up to ensure patients remain engaged in their treatment and services.

Qualifications:

  • Education: High school diploma required; Bachelor’s degree in Human Services, Social Work, Psychology, or related medical field preferred.
  • Experience: Experience in community health, healthcare, or a related field, with a focus on Substance Use Disorders, mental health, and/or medical conditions.
  • Knowledge: Understanding of substance use disorders, mental health challenges, community health resources, and common medical conditions.
  • License: Valid driver’s license and reliable transportation to assist with patient transport.
  • Skills: Excellent communication skills and the ability to build rapport with diverse populations. Ability to work effectively in a team setting.

Dental Front Office Receptionist (Dental Office Experience Required)Tazewell, VA

Key Responsibilities:

  • Greets patients and answers telephone in a courteous and professional manner, addresses questions.
  • Ensures patient confidentially, and provides patients with needed information.
  • Schedules/reschedules appointments and maintains smooth patient flow throughout dental clinic.
  • Gathers primary, secondary, and third party payment information.
  • Ensures that each patient is advised of the Sliding Fee Scale and makes adjustments accordingly.
  • Collects and receives patient co-pays as applicable. Advises patients of any outstanding balances and directs to billing office as needed.
  • Collects and maintains accurate patient registration and demographic data at each visit.
  • Redirects calls to appropriate dental staff.
  • Scans information into the appropriate patient’s documents in the electronic health record.
  • Sorts and distributes incoming mail or faxes to appropriate staff. Assists in faxing outgoing documents as needed.
  • Provides receipt, visit summary, and next appointment to all patients at the end of the visit, and if applicable, work/school excuses as directed by provider.
  • Assist patients with dental records releases or other forms as needed.
  • Performs other necessary duties required by the site manager, senior management, or CEO/Executive Director to meet the goal of providing primary health care.

Qualifications

  • High school diploma or equivalent. Associates degree in health care management, business management, or related field preferred.
  • Experience in a dental office.
  • Proficient with Microsoft Office products including Word, Excel, and Outlook.
  • Ability to maintain confidentiality.
  • Ability to multi-task and deal with stressful situations.
  • Ability to communicate in person, electronically, or on the phone with staff, patients, and general public.

Dental Care Coordinator (Dental Office Experience Required)Tazewell, VA

Key Responsibilities:

  • Serve as the primary point of contact for outside referral sources, dental patients, answering questions, providing information, and assisting with scheduling.
  • Coordinate dental appointments, ensuring patients are scheduled promptly and accurately.
  • Maintain accurate patient records, including dental charts, treatment plans, and progress notes.
  • Assist with managing and updating referral network.
  • Follow up with patients to ensure referred treatment is completed.
  • Educate patients on oral health practices, insurance benefits, and treatment options.
  • Ensure that patients receive reminders for upcoming appointments and follow-up care.
  • Coordinate with other departments and healthcare providers to ensure comprehensive patient care.

Qualifications:

  • High school diploma or equivalent; associate degree or higher in a healthcare-related field preferred.
  • Previous experience in a dental office required.
  • Knowledge of dental terminology, insurance processes, and scheduling systems is required.
  • Strong organizational skills with the ability to multitask and prioritize in a fast-paced environment.
  • Excellent communication and interpersonal skills to work effectively with patients, providers, and staff.
  • Proficient in Microsoft Office Suite (Word, Excel, Outlook) and EMR systems.
  • Ability to maintain confidentiality and adhere to HIPAA standards.

Pediatrician – Tazewell

The Pediatric staff physician works as part of the medical provider team providing medical services to the pediatric patients of SVCHS. In addition, staff physicians, unless specifically exempted by the CMO, shall be qualified to serve on the active medical staff of a local hospital, assume “on call” after hours responsibilities and supervise community health center PAs or NPs as appropriate.

Qualifications:

– Graduation from an accredited medical school with a MD or DO degree.

– Completed residency and Board Certification preferred.

– Licensed in VA.

– The ability to relate with warmth & effectiveness to patients and other health care providers of

SVCHS.

– Must be ACLS qualified.

– Ability and willingness to serve on the active medical staff of a hospital used by SVCHS.

Remote Behavioral Health Therapist

Duties:

The Remote Behavioral Health Therapist will provide assessment and treatment of patients and/or families with BH concerns via telehealth and in person as needed. The Telebehavioral Health Therapist will provide consultation to primary care providers regarding patients’ integrated behavioral health needs.

– Assists the primary care provider in recognizing, diagnosing, treating, and BH issues as part of an integrated care team.

– Provides behavioral health screening, assessment, and outpatient treatment including individual, group, and family modalities.

– Utilize telehealth technology to provide services and in person when needed.

– Helps patients reduce symptoms and improve coping skills related to chronic medical conditions such as diabetes, chronic pain, and hypertension.

– Provides integrated care through treatment planning, team meetings, and effective communication with team members.

– Documents behavioral health services in accordance with policies and procedures regarding medical records.

– Follows confidentiality laws and policies.

Skills:

· Skills in evidenced based behavioral health assessment, treatment and crisis intervention.

· Ability to effectively communicate behavioral health issues with other treatment team members, patients, and families.

· Skill in preparing and maintaining patient records and to confidentially manage patient information.

Requirements:

· Master’s Degree in Social Work, Counseling, or Psychology.

· One (1) to three (3) years of experience in the health care setting.

· Licensed mental health practitioner certified to practice by the Virginia Board of Health Professionals (ie. LCSW or psychologist)

· Excellent working knowledge of behavioral medicine and evidence based treatments for medical and mental health conditions.

· Knowledgeable in telehealth technologies.

· Must be qualified in Basic Life Support techniques and CPR.

Behavioral Health Supervisor – Meadowview

Duties:

Works as part of an integrated medical team of health professionals in a primary care setting. Provides individual and family brief and long term therapy for behaviors related to chronic health conditions as well as serious mental illnesses. Provides consultation to primary care providers regarding patients’ behavioral health to include screening, assessment, diagnosis and treatment recommendations. Telehealth and in person services with one work day from home. Supervises BH Care Manager.

Skills:

· Skills in evidenced based behavioral health assessment, treatment and crisis intervention.

· Ability to effectively communicate behavioral health issues with other treatment team members, patients, and families.

· Skill in preparing and maintaining patient records and to confidentially manage patient information.

Ability to use independent judgment.

Requirements:

· One (1) to three (3) years of experience in the health care setting.

New Day Recovery Program Coordinator (Tazewell)

Job Overview:

Come work full time for a Substance Use Disorder recovery program that provides comprehensive care with therapy, case management, and MOUD/MAUD services. Work 10 hours Monday-Thursday and enjoy 3 day weekends. The position provides clerical support, site management, patient reception, scheduling and behavioral health support for patients & families impacted by substance use disorder.

RN/LPN Care Coordinator

Job Overview
We are seeking a dedicated Registered Nurse or Licensed Practical Nurse to join our healthcare team. Want to do meaningful work yet have a life too? No 12 hour shifts, no weekends. The ideal candidate will have experience primary care settings and working on multidisciplinary healthcare team, with the ability to provide exceptional patient care.

Duties
-The candidate will work alongside the providers and office staff in a Patient-Centered Medical Home (PCMH) environment, potentially serving multiple locations.

-The candidate will provide care coordination services for patients and assist the practice to implement systems changes to improve quality and access to care, and reduce hospital admissions, ER visits, and overall healthcare costs.

-The candidate will actively participate in multidisciplinary patient-centered team meetings and develop and strengthen community partnerships.

-The candidate will assist providers and other care team members in implementing processes for best practices for preventive services, chronic care and disease management.

-The candidate will utilize EMR /registries reporting to identify high-risk patients, proactively engage patients, ensure they maintain a connection to the medical home, conduct pre-visit planning with providers such as printing daily huddles sheets and risk assessments and prioritize patient follow-up.

-Work collaboratively with providers and the care team to ensure development of the plan of care in partnership with the patient and family, including all appropriate preventive and disease-specific screenings, interventions, treatment goals including self-management goals
– Collaborate with multidisciplinary teams to ensure comprehensive patient care
– Utilize electronic health record systems such as eClinical Works for accurate documentation

Qualifications
– Must hold a valid RN or LPN license in the state of Virginia

-Certified Case Management (CCM) certification. (Preferred)
– Minimum of 5 years experience in a health care setting.
– Ability to travel for assignments if required
– Strong communication and interpersonal skills
– Knowledge of conducting physical examinations and interpreting medical information

-Discharge planning or related experience.

Migrant Health Outreach Worker

Job Summary
Southwest Virginia Community Health Systems is an organization made up of Federally Qualified Health Centers that serves the area of rural Southwest Virginia. We have been serving patients for over 40+ years and are currently seeking a Migrant Health Network Outreach Worker to join our team working in Washington/Smyth Counties, VA.

Duties
-Serve as intermediary between the community health centers and farm workers. Work independently to located farm workers and provide case management, health education, transportation, and translation services. Maintain positive relationships with growers, farm workers, and clinic staff. Must document all activities as performed.

Outreach

– Locate farm workers throughout the jurisdiction to which assigned and complete health assessments at each location.

– Inform farm workers of clinic services and other community services.

– Compile jurisdiction information (grower list, community agency contacts, etc.) in Continuity File to assist future staff.

  • Case Management

– Interview farm workers to determine needed services.

– Provide case management by assisting farm worker to obtain needed services in community through referrals, phone calls, and interpretation, translation, and advocacy activities.

– Seek methods of transportation and translation to assist farm worker to obtain needed assistance. These services may be provided by outreach worker if other resources are not available.

  • Health Education

– Provide health education at farm worker location or in clinic setting.

– Teach or organize pre-natal sessions as needed.

Qualifications
– Must have Qualified Medical Interpreter or Bridging the Gap certificate or equivalent.

  • Three (3) years of experience working with social services programs.
  • Extensive knowledge of program development and implantation.
  • Thorough knowledge of state and local social service resources.
  • Thorough knowledge of Medicaid reimbursement procedures and awareness of other third-party reimbursements.
  • Thorough knowledge of medical record requirements, social service standards and staff training resources.
  • Ability to establish rapport and relate to clients, families, community agencies and groups, and variety of service disciplines.
  • A working knowledge or understanding of substance abuse issues.
  • Must be qualified in Basic Life Support techniques and CPR.
  • Must have a valid driver’s license.
  • Must be a U.S. citizen
  • Must be bilingual (Spanish/English)
  • Must have good driving record.

Nurse Practitioner – New Day Recovery/Tazewell, VA

Job Overview:

The nurse practitioner works as part of the clinic provider team performing a variety of direct patient services including assessment, diagnosis, implementation of treatment, evaluation, and monitoring the general patient management plan for clinic patients. The nurse practitioner performs these functions under the general supervision of a licensed physician in a primary health care setting. This role provides services to the New Day Recovery program, which is an office-based addition treatment site. This includes MOUD/MAUD and PCP treatment as part of an integrated treatment team.

Qualifications:

  • Graduation from an accredited school of nursing, a nurse practitioner program recognized by the Board of Nursing or from a program otherwise designated for Family Nurse Practitioners which has been approved by the state Board of Nursing.
  • The ability to relate with effectiveness to the patients and other health care providers of SVCHS.
  • Must be qualified in ACLS.
  • Sufficient experience to carry out the duties of this position.

Peer Recovery Specialist (Saltville, Bristol)

The Peer Recovery Specialist’s role is to support others in recovery from a substance use disorder. The Peer Specialist will serve as a role model, mentor, advocate, and motivator to recovering individuals in order to help prevent relapse and promote long-term recovery. The Peer Specialist must demonstrate an ability to share personal recovery experiences, to instill hope, and to develop authentic peer-to-peer relationships. The Peer Recovery Specialist will serve as a member of the interdisciplinary New Day Recovery team and provide support to the peer recovery center. The CEO/Executive Director and the Governing Board reserve the right to adjust schedules and transfer any staff member within the organization, at any time for the benefit of our patients and the overall operations of Southwest Virginia Community Health Systems, Inc.

Qualifications:

  • 2 years of Healthy Recovery
